Antiphospholipid
syndrome (APS) is an autoimmune disease with various clinical
manifestations, including arterial and venous thrombosis,
thrombocytopenia, recurrent fetal loss and livedo reticularis as
well as cardiac, hematological and neurological manifestations. In
its worst manifestation, APS presents as the so-called catastrophic
APS with disseminated thrombus formation in a variety of vessels
leading to multi-organ failure and thrombocytopenia.
APS is caused due to
Antiphospholipid antibodies which comprises of a heterogenous group
of autoantibodies primarily described as antibodies directed
against various negatively charged phospholipids.
Two major serological
parameters for diagnosis, anticardiolipin and anti-ß 2-GPI
antibodies, have been discovered quite some time ago and are well
established. There is, however, increasing evidence from a number of
studies that testing a profile of various phospholipids apart from
cardiolipin and ß 2-GPI increases both the sensitivity and
specificity of the laboratory diagnosis.
Looking at the general
composition of human cell membranes based upon six phospholipids it
appeared obvious to check for autoantibodies against these,
particularly as the distribution of these phospholipids strongly
differs within cell compartments and cell types. Despite some
contradictory results, a number of studies indicates that there is a
sub-group of APS patients with a clear clinical picture of APS but
without detectable cardiolipin / ß 2 -GPI antibodies or LA. However,
other phospholipid antibodies can be detected in the serum of these
patients, proving phospholipid profile and screening tests to be an
important additional diagnostic tool.
